Vaccine Scheduling and Consent Forms

Spring Park Pharmacy is a one-stop shop for all of your immunizations. Our pharmacists are trained to administer a variety of vaccines and, of course, are CPR certified. Each vaccination is documented in the Minnesota Immunization Information Connection (MIIC), which is available for all of your health care providers to view. We can also review your immunization records with you to determine if you are due for any other preventative immunizations to ensure you stay as healthy as possible. Oftentimes, immunizations are free or cost less than your doctor’s office.

Each fall and winter, we offer the following vaccines by appointment:

• Influenza shots
• Influenza High Dose (65+ years old)
• COVID-19
• RSV (for those who qualify)

The following immunizations are available year-round.:

• Tetanus, diphtheria, pertussis (Td & Tdap)
• Pneumonia (Prevnar 20)
• Shingles (Shingrix)

Our pharmacists administer the influenza vaccine to patients 6+ and other CDC recommended vaccines to patients 13+. All vaccinations are subject to insurance coverage.
